Terms of Agreement and Policies for Integrity Health Source, LLC: Summer 2025 NBE Prep Mentorship Group

1. Purpose: The purpose of this mentorship group is to provide support, guidance, and resources to certified health coaches as they prepare for the board certification exam.

2. Membership: Membership in this group is voluntary and does not guarantee passing the board certification exam.

3. Large & Small Group Participation:

• Active participation in group discussions and activities is encouraged.

• Positive & respectful communication is essential.

4. Confidentiality: All group discussions and shared information should be kept confidential.

5. Liability: The mentor and group leaders are not liable for any outcomes or results, including passing the board certification exam.

6. Termination: Membership may be terminated at any time and no refunds will be given for services not yet rendered.
